If you were certified during any portion of 2008 (even if the certification expired in January or a new certification was issued in December), you must still submit a 2008 PRL report. Every commercial permittee is required to file an annual sales report, even if they did NOT make sales last year. Reports are due no later than February 1. Reports may be filed now or anytime during January; there is no need to wait until February 1. If an applicator, technician, aquatic anti-fouling paint applicator or commercial permittee receives a LATE notice, DO NOT ignore this notice. We either did not receive the report or there was a problem with the report. Contact the Pesticide Reporting & Certification Section at prl at gw.dec.state.ny.us to resolve the problem. Use your DEC issued pesticide applicator ID card to obtain the correct ID number (many people record their ID number incorrectly and do not get credit for submitting their report). Commercial Permit numbers begin with the letter R. Commercial permits are issued to entities that sell restricted-use pesticides. If you wish to email your report as an attachment, please email to: prlfiles at nysprl.com Electronic reports that are received by e-mail should receive two responses. Save these responses! The first response will be an automatic response saying a report was received. The second response should contain a confirmation number. All electronic reports are issued a confirmation number. Keep this number for your records, this is your PROOF that you sent an annual report and it was received. If you do not receive the second email response, then there is a problem with your report and it may NOT have been received. Applicators, technicians and/or commercial permittees think they do not have to file if they did not many any applications or sales. This is not true. You must still submit an annual report even if you did not many any applications or sales in 2008. Applicators or technicians thought that the company, business or agency would file for them. The business is NOT required to submit an annual report for you. If the business chooses to report on behalf of their applicators, that is fine, but they are not required to do so. The responsibility for submitting an annual PRL report falls on the applicator or technician. Applicators, technicians and/or commercial permittees did not get the reminder postcard. You must still submit a report. We mail out reminder notices as a courtesy. However, if you do not receive a reminder, it is still your responsibility to submit a report on time. Please make sure we have your current address on file. The wrong ID number was listed on the report. Often times, someone will transpose a digit in their ID number and they do not get credit for reporting. Please double check to make sure your ID number is correct on your report before sending it in. The report was sent to the wrong address. DEC moved to downtown Albany in 2001. We are no longer at 50 Wolf Road in Albany. Do not mail your reports to any of the DEC regional offices. Many handwritten reports are not LEGIBLE. If we cannot read the name or ID number on the report, we cannot give you credit for submitting a report. Please make sure your reports are neat and your handwriting is clear so we can read it. There are many problems with EPA Registration Numbers. The numbers are written incorrectly, hyphens are missing, a number is omitted, EPA establishment numbers are used instead of the EPA product registration number. Please make sure you have the correct EPA registration number for the product used (compare it to the label). Many people list products that are not pesticides like glue boards, mouse traps, and straight fertilizer. If the product is a pesticide, it should contain an EPA registration number on the label. Many people list a unit of measurement that is not correct. For instance, (bag, each, piece, pack, chunk, G). You must use one of the approved units of measurement. They can be found on the instructions for the form or in the electronic programs on our website. Many times, the County code is omitted, or letters are used instead of the number. You must list the two digit county codes that are approved by DEC. They can be found on the instructions for the form or in the electronic programs on our website. Some people do not list the zip code or list the wrong zip code for the associated address. Please make sure you list the 5 digit zip code and that it is the correct zip code for the address where the product is applied or sold. Ofte times, reports that are emailed do not contain contact information, and often times they do not list the business registration number or applicator certification identification numbers. Please make sure you list your name, certification ID numbers, business registration name and number or commercial permit name and number, and a telephone number and email address where we can contact you if there is a problem with your report.
